We have developed an online survey.   This survey walks you through 5 areas.  A summary of each section is provided below.   When you are ready to take the survey please click on “Start Survey”

Section 1: Demographics
Demographics Description
In this section, the survey will gather demographic data.   The purpose of the demographic data is to develop an organizational structure, identify departments, identify managers and identify roles within the organization. Although demographic data will be recorded, responses to questions will not be identified in our reporting such that specific individuals’ positive, neutral, or negative responses will be uniquely identified. Your input is anonymous.

Section 2: Work and Performance
Work and Performance Description
In this section, the survey will gather each person’s view on various aspect of their input, performance and outcomes.    The survey also will gather data on organizational performance.

Section 3: Perceived Motivation
Perceived Motivation Description (For Employees)
In this section, the survey focuses on motivational forces in action at the organization.    There may be motivation forces that are organization wide, department wide, or manager specific.    The following questions will be based on a scale.   This data will be aggregated to determine common themes of a manager, department, or organization.   The questions in this section are what you perceive the motivation forces your direct supervisor, department manager and CEO are using.

Section 4: Applied Motivation
Perceived Motivation Description (For Managers)
In this section, the survey focuses on motivational forces in action at the organization.    There may be motivation forces that are organization wide, department wide, or manager specific.    The following questions will be based on a scale.   This data will be aggregated to determine common themes of a manager, department, or organization.   The questions in this section are what you think you do when motivating those under your direct supervision.    If you do not supervise any employees, please skip this section.

Section 5: Volunteer Motivation
Volunteer Motivation Description (For Managers)
In this section, the survey focuses on motivational forces in action at the organization. There may be motivation forces that are volunteer specific. The following questions will be based on a scale. This data will be aggregated to determine common themes of a volunteer within an organization. The questions in this section are what you perceive the motivation forces your volunteer coordinator is using. If you are not a volunteer, please skip this section.

Section 6: Open Response
Open Response Description
In this section, the survey provides an avenue to leave responses to general questions and allow for the input of data not asked.
